* Rules:

1. Referral must be a valid Ambulatory Surgery Center, Hospital or Clinic.
2. Referral cannot be a current customer of JCB Laboratories.
3. One 10% discount coupon will be awarded for each qualified referral.
4. Each coupon will expire 90 days from the date of issue.
5. If referral places an order with JCB Laboratories within 90 days, the referral source will receive a 10% discount for the next 6 months or 6 orders, whichever occurs first.
